1. Since the EPA hasn't set any standards

They leaned on CARB..they have been setting standards stricter than the Feds for decades. Look at old pictures of LA. Many states followed their lead so manufacturers made CA compliant cars.
The vast majority of drivers don't do any engine modifications, especially these days. Mostly it is all for looks. 5 percent of cars pumping out more exhaust is still a big number in a state like CA. Plus there are still a fair number of vehicles on the road that come nowhere near todays standards.
This administration is also trying to het rid of CAs exemption allowing the state to enforce stricter exhaust standards.
More pollution, more smog, more deaths.
